This application is open to all individuals with I/DD and their caregivers in the state of Colorado. Community Roots Foundation primarily provides one-time or short-term support for individuals with I/DD experiencing financial barriers. Each application will be reviewed with care, confidentiality, and a person-centered lens.

Funding decisions are based on:

• Demonstrated need

• Impact on quality of life and well-being

• Alignment with the Foundation’s mission

• Available funding resources

The Foundation does not provide direct cash assistance and generally pays vendors or service providers directly. Some requests may require additional documentation or board review.
